You will join a dynamic service that takes a preventative and whole systems approach to tackling childhood obesity and health inequalities at a time when supporting disadvantaged members of our society is more essential than ever.

As a Family Coach, you’ll be passionate about transforming the lives of children, young people and their families aged 5-19 (or up to 25 with Special Educational Needs and Disabilities) to lead healthier, more active lives. You’ll deliver engaging, tailored, evidence-based health and wellbeing programmes through one-to-one sessions and group-based settings, empowering children, young people and families to make informed and sustainable lifestyle choices.

You will be passionate about delivering personalised support in community spaces, schools and homes across Kensington and Chelsea and Westminster City Council, building trusted relationships and consistently promoting the Change4Life health messages.

You will also join a dynamic, preventative service that takes a whole-systems approach to tackling childhood obesity and health inequalities – at a time when supporting disadvantaged members of our society is more essential than ever. This is a unique opportunity to contribute to an innovative, borough-wide initiative that blends holistic health promotion with community-based engagement. This role is ideal for someone with experience in behaviour change, nutrition, and physical activity, who thrives in a person-centred, empowering environment.

Main Responsibilities
• Deliver personalised one-to-one sessions with children, young people, and families, focusing on nutrition and physical activity.
• Support families to identify their needs and co-create realistic, achievable health and wellbeing goals.
• Support the delivery and coordination of Change4Life Clubs.
• Adapt approaches and communication styles to meet the needs of children and young people with SEND, ensuring inclusivity and accessible support.
• Work collaboratively with local partners, schools, school nurses, children’s centres and youth organisations.
• Build strong, trusted relationships with families to encourage behaviour change.
• Maintain accurate records of assessments, sessions and monitor impact trough child and family voice.
• Ensure all safeguarding, health and safety, data protection and lone-working policies are followed at all times.

Main Requirements (for details check the job description and person specification)

Qualifications/Education:
• NVQ Level 3 or equivalent experience in health, education, social work, or equivalent experience with children and young people and health and wellbeing promotion.

Experience:
• Experience of delivering health promotion programmes of support to facilitate sustained behaviour change to health and wellbeing with knowledge and experience of delivering group and 1:1 evidence-based interventions to support families.
• Experience of working in multi – disciplinary teams and ability to build, develop and maintain effective working relationships with key stakeholder especially staff in Schools, Children’s Centres and Family Hubs.
• Experience of taking a whole family approach, agreeing targets in consultation with children, young people and their families and monitoring, evaluating and measuring progress.
• Experience working with children and young people with SEND.
• Experience working in group settings.

Knowledge/Skills:
• Understanding of preventative strategies, health inequalities/barriers encountered by children's young people and families and of the wider social factors (determinants) that affect people's well-being – specifically those in Kensington and Chelsea and Westminster.
• Understanding of Trauma Informed practice and understanding of Adverse Childhood Experiences and its relationship to health and wellbeing.
• Good record keeping skills and confidence using database systems to accurately record, monitor and report on activities and outcomes.
• Strong communication, facilitation and interpersonal skills.
